Transition Coaches and Centres
A Transition Coach is a consistent point of contact throughout a transition journey. They provide advice, and work with members and their families to design a personalised transition plan.
A Transition Coach can be accessed by contacting a Transition Centre. All members must transition through a Transition Centre regardless of length of service or mode of transition. The national network of Transition Centres are located on or near most major bases.
Defence Force Transition Program
Defence Force Transition Program (DFTP) provides advice, support and programs to ADF members to assist with transition. DFTP services and programs are accessible for 24 months after separation. The following factsheet and video provide further details.
