• DEC01 - Export Application Form (PDF - 245kb)
  Form used to apply for a permit or licence for all exports of Defence and related goods listed in the Defence and Strategic Goods List.
  Note that while this form may be completed on screen, it must be signed and can only be sent by mail or fax.
• DEC02 Instructions for completing Form DEC01 (PDF - 141kb)
  Please read first before completing Form DEC01 above.
• DEC03 - Statement by Ultimate Consignee or Purchaser (PDF - 190kb)
  Form used by an ultimate consignee to verify that dual-use goods exported from Australia will only be used for the specified purpose by the specified end user listed in Form DEC01. When used, this form accompanies Form DEC01.
• DEC04 - End User and Non-Transfer Certificate (PDF - 51kb)
  Form used by an Australian Company (or Defence Project) seeking to export military goods from Australia. This certificate provides an undertaking by the relevant end-users that they will use the goods for the purposes indicated and will not resell or transfer the goods without the written permission of the Australian Minister for Defence or his/her delegate.
• DEC05 - Application for International Import Certificate (IIC) (PDF - 165kb)
  Form used by Australian importers to apply for an IIC to verify to an overseas government that the goods to be imported into Australia will be subject to Australian export controls.
• DEC06 - Application for Delivery Verification Certificate (DVC) (PDF - 132kb)
  Form used by Australian importers to apply for a DVC that is required to verify to an overseas government that goods have entered Australia and are subject to Australia's export controls.
• DEC07 - Restricted Goods Permit (RGP) Application (PDF - 90kb)
  Form used by non-commercial firearms exporters. This form is provided for use by Australian and non-Australian residents to export specified firearms which are carried as accompanied luggage. This form must be made available to Customs at the international point of departure for authorisation and verification of the firearms prior to export. Please read the conditions of use contained on the form to ensure it is applicable to your export purposes. This application form must be presented to customs prior to departure.
• DEC09 Firearm/Ammunition End Use Certificate (PDF - 50kb)
  Form used by end users to verify that firearms/ammunition being exported to private individuals overseas have the approval of the Police Authorities in that country.
• US End Use Certificates (DSP-83)
  Form requested by the U.S. government for authorisation to export significant military equipment and classified equipment or data. The form must be completed by the appropriate foreign persons (eg, consignee, end-user,government) and forwarded to the US Department of State through the US person making the application.
