Use this form to apply for a technical assessment of your goods or services against the Defence and Strategic Goods List and the Weapons of Mass Destruction Act. - DEC05 - Application for International Import Certificate (IIC) (PDF - 165kb)
Australian importers please use this form to apply for an IIC to verify to an overseas government that the goods to be imported into Australia will be subject to Australian export controls.
Note: This form is not an application for an Import Permit. It is the responsibility of the importer to refer to the Australian Customs & Border Protection Service (http://www.customs.gov.au/) and relevant State Regulations on whether an import permit is required. - DEC06 - Application for Delivery Verification Certificate (DVC) (PDF - 132kb)
Australian importers please use this form to apply for a DVC where you are required to verify to an overseas government that goods have entered Australia and are subject to Australia's export controls. - DEC07 - Restricted Goods Permit (RGP) Application (PDF - 90kb)
Non-commercial firearms exporters please use this form. Australian and non-Australian residents wishing to export specified firearms which are carried as accompanied luggage must present this form to Customs at the international point of departure for authorisation and verification of the firearms prior to export. Please read the conditions of use contained on the form to ensure it is applicable to your export purposes. This application form must be presented to customs prior to departure.

This form is used by an ultimate consignee to verify that dual-use goods exported from Australia will only be used for the specified purpose by the specified end user listed in Form DEC01. When used, this form accompanies Form DEC01. - DEC04 - End User and Non-Transfer Certificate (PDF - 51kb)
This form is used by Australian Companies and Australian Government agencies including the Department of Defence and the Defence Materiel Organisation when exporting from Australia all military goods controlled under the DSGL. This certificate provides an undertaking by the end-users of the military goods that they will use the goods for the purposes indicated and will not resell or transfer the goods without the written permission of the Australian Minister for Defence or his/her delegate. - DEC09 Firearm/Ammunition End Use Certificate (PDF - 50kb)
End users must use this form to verify that firearms/ammunition being exported to private individuals overseas have the approval of the Police Authorities in that country. - US End Use Certificates (DSP-83)
The U.S. government requires this form for authorisation to export significant military equipment and classified equipment or data. The form must be completed by the appropriate foreign persons (eg, consignee, end-user, government) and forwarded to the US Department of State through the US person making the application.
