PM031 Department of Defence Prescription Forms for Dispensing by Community Pharmacists

In the case of an emergency or a clinically essential circumstance, a prescriber may authorise supply of medication for an Australian Defence Force member from a civilian pharmacy using the Department of Defence PM31 Prescription Form. The following restrictions apply to dispensing from a PM31:

a. the quantity of any drug supplied is to be limited to one month supply or closest original pack;

b. only medicines may be supplied (not dressings or medical appliances); and

c. repeats are not authorised.

Action by Community Pharmacist

Only where the prescription has been endorsed "Authorised to be Dispensed by Community Pharmacist" can it be dispensed and supplied free of charge to the patient.

Retain the duplicate copy.
