PM031 Department of Defence Prescription Forms for Dispensing by Community Pharmacies

In the case of an emergency or a clinically essential circumstance, a prescriber may authorise supply of medication for an Australian Defence Force member from a community pharmacy using the Department of Defence PM031 Prescription Form. The following restrictions apply to dispensing from a PM031:

a. the quantity of any drug supplied is to be limited to one month supply or closest original pack;

b. only medicines may be supplied (not dressings or medical appliances); and

c. repeats are not authorised.

Action by Community Pharmacy

Only where the prescription has been endorsed "Authorised to be Dispensed by a Community Pharmacy" can it be dispensed and supplied free of charge to the patient.

Retain the duplicate copy.
