Conditions of Use - Registration – ADF Families

To register eligible dependants for the trial, ADF members should perform the following activities:
- Read the Conditions Of Use contained within the Registration Form (AD858-1 ),
- Ensure dependant details are current and accurate in PMKeyS (ie Name, Address, Date of Birth and Gender). If dependant details are incorrect or incomplete:
- submit an AD160 Dependant Details to your Admin Officer or Customer Service Centre
- Confirm your PMKeyS Dependant Details have updated correctly
- Check dependant details are correct with Medicare Australia? ( ie Name, Address, Date of Birth, Gender and that your Medicare Card is current and active)
- Complete the Form AD858-1 ADF Family Health Registration, ensuring information is consistent with Medicare Australia and PMKeyS.
- Ensure each Dependant Registration form has the appropriate Dependant consent provided.
- Fax the form to the ADF Family Health Directorate: (02) 6266 3784; or

Upon successful registration, you will be provided with an ADF Family Health Card which will be used for accessing the benefits associated with this trial. Participation in the trial does not commence until the card has been received.
Opt in/Opt out
- ADF members may elect to opt in or opt out their dependants from the trial at any time.
- Dependants will not be eligible to receive the benefits associated with this trial until the registration process has been completed, and the ADF Family Health Card received.
- All members who elect to deregister a dependant must complete form AD858-3 ADF Family Health Trial Cancellation and until such time as this form has been received and processed by ADF Family Health, dependants may continue to receive benefits. A letter confirming cancellation will be sent by ADF Family Health, requesting that the ADF Family Health Card be destroyed.
- ADF members may register or deregister their eligible dependants for either the medical or dental provision, or for both.
