Before completing your AD858-1 Family Health Trial form, have you:
- read the Conditions Of Use?
- ensured that your dependant details are current and accurate in the Dependant/Beneificary Section of PMKeyS (ie Name, Address and Date of Birth)? If dependant details are incorrect or incomplete:
- submit an AD160 Dependant Details to your Admin Officer or Customer Service Centre
- Confirm your PMKeyS Dependant Details have been updated correctly
You should then:
- Complete the Form AD858-1 ADF Family Health Form, ensuring information is consistent with PMKeyS.
- Ensure each Dependant Registration has the appropriate Dependant consent provided.
- Fax the form to the ADF Family Health Directorate: (02) 6266 4292; or

When your AD858-1 ADF Family Health Trial Form is received by ADF Family Health staff you should expect a confirmation e-mail within 2 days.
Upon successful registration, you will be provided with an ADF Family Health Card which you will use to access the Allied Health benefits associated with this trial. Participation in the trial does not commence until the card has been received.
Deployment and Registering Dependants
