Conditions of Use - Notifying Changes

Members are responsible for ensuring the accuracy, currency and completeness of all registration information including Medicare numbers. Any changes must be immediately notified to the ADF Family Health (ADFFH) Directorate.
- Changes that impact eligibility, that are not notified and which result in the processing of a benefit, will be treated as a breach of the Conditions of Use.
In order to ensure the security of personal information, and to adhere to Defence's policy not to duplicate data, the information you provide when registering for the trial will be managed by three separate areas. When notifying changes it is important to note the responsible areas as described below:
1. PMKeyS Information: PMKeyS is the source for updating your Dependant Details. This information will be extracted from PMKeyS for the purpose of creating pre-populated registration forms, and for registering eligible dependants. Dependant Details must be updated within PMKeyS using the AD160 form. This includes:
- Dependant Name;
- Dependant Address, City, State and Postcode;
- Dependant Gender;
- Dependant Date of Birth; and
- Adding a new dependant.
In order for Medicare Australia to identify dependants as participating in the trial, dependants Medicare records must match the Dependants Details provided in PMKeyS, and the Medicare card details that have been provided to ADFFH. If this information does not match you will not be registered for the medical component of the trial, even if you have received an ADF Family Health card. Your medical practitioner will not receive the expected payment from Medicare for the services provided and will be entitled to request payment from the dependant for any amount outstanding.
2. ADF Family Health - HealthKEYS Database: The following Information provided through the Registration process will be captured within the HealthKEYS Database and will be maintained by the ADFFH Directorate, all amendments to the this information must be notified to ADFFH using the AD858-2 form. This includes:
- Dependant's Medicare Number and Reference Number;
- Primary Contact Name;
- Mailing Address, City, State and Postcode;
- Telephone number;
- Email address;
- Opt-In for Medical and/or Dental - Start Date;
- Opt-Out for Medical and/or Dental - End Date; and
- Consent provided by Dependant who has turned 14 years of age and has the capacity to provide consent to continue registration in the trial.

3. Medicare Australia - Medicare Australia will receive information from Defence to support the medical component of the trial. All information supplied by Defence will continue to be maintained by Defence, therefore, changes must be advised as per 1 and 2 above.
In order for Medicare Australia to identify dependants as participating in the trial, dependants’ Medicare records must match the Dependants Details provided in PMKeyS, and the Medicare card details that have been provided to ADFFH. If this information does not match you will not be registered for the medical component of the trial, even if you have received an ADF Family Health card. Your medical practitioner will not receive the expected payment from Medicare for the services provided and will be entitled to request payment from the dependant for any amount outstanding.
However, Medicare Australia will need to be advised if any of the following changes occur:
- New baby needs to be added to existing Medicare card;
- An additional Medicare card is required, or an old one replaced;
- If there are any name changes; and
- If there is a change of address: notify Medicare Australia and PMKeyS (form
AD160). You will also need to advise the ADFFH (form AD 858-2) if it affects your mailing address as indicated upon registration..
4. Navy Health Ltd - Navy Health will provide claims processing, benefit payment, customer service and card production. To support their customer service, Navy Health may seek additional information. This additional information is not shared with Defence.
ADDITIONAL NOTE: Dependants registered for the program, upon reaching the age of 14 years and having the capacity to provide consent, will be expected to provide their consent. Our system will periodically determine whether minors have reached the age of 14 and notification will be sent to request update of registration and consent.
