How to make a Freedom of Information request: Ex Serving Member

The following form is to be completed and signed before being sent directly to the relevant Service personnel office or civilian personnel authority.
Requests from ex-service personnel seeking copies of their own records are to contain:
- Personal details - full name, date of birth, service number and date of discharge (if known). These details are required to assist in identification of the correct service records.
- Signature of person making the request
- Purpose of the request
- Postal address of the person making the request
Navy Service Records
If you are requesting service records of an Australian member who served during World War I and World War II it is recommended that you contact the National Archives of Australia (NAA) in the first instance.
