PMKeyS Forms

The following electronic forms have been developed to enable ADF personnel administered by ADFPC-Canberra to change their PMKeyS personnel information via email. Simply click on the links below and fill in the required form.

Defence Publishing Service Web Forms System - PMKeyS Forms

ADF Personal Details - (Form AD150). This form is used to record a range of data including information about your emergency contact, next of kin, nominated family for leave travel, and details about your dependants.

The following forms are to be submitted when changes occur in the members personal circumstances as follows:

ADF Change of Address Details - (Form AD149). This form is used to record a members change of address details. If the change of address occurs because a member's personal circumstances change i.e. Marriage/Separation/Divorce, then the member will need to complete the AD150 ADF Personal Details .

ADF Emergency Contact Details - (Form AD159). This form is used when ADF Personnel wish to change the details for the Primary and Secondary persons they wish to have contacted in the event of an emergency.

ADF Dependant Details - (Form AD160). This form is used when ADF Personnel wish to change the details of their dependants. Either their spouse, children or other.

Change in Marital Status - Members changing their marital status should seek advice from the ADFPC-C personnel services section as to what action to take. Supporting documentation e.g. marriage certificate or divorce papers will need to be provided. Whenever there is a change to your status, the following form is to be completed and forwarded to ADFPC-C Personnel Section:

Categorisation Form (This form is to be printed and then forwarded to ADFPC-C)

If the application is being completed by divorced or separate members who have overnight custody of dependant children and wish to gain an entitlement to a MQ or RA then Part 2 of the application MUST BE completed

Recognition of Dependent for Purpose of MQ Acquisition (This form is to be printed and then forwarded to ADFPC-C) Change in Number of Dependants. Personnel should visit the ADFPC-C with supporting documentation e.g. birth certificate /recognition of dependants approval advice for the necessary reporting action to be undertaken.

ADF Nominated Family for Leave Travel - (Form AD290). This form is used by ADF Personnel entitled to an ADF travel entitlement. As this entitlement needs to be approved by CO ADFPC-C, amplifying comments may need to be provided.

ADF Employment Preferences and Restrictions

Form AD148 - ADF Employment Preferences and Restrictions is computer generated and available through the Web Forms System. It is intended that the form be completed electronically. This form is to be completed by ADF Personnel (optional for RAN Officers), along with the ADF Performance Appraisal Form. ADF personnel may complete the form at any time to alter their Employment Preferences and Restrictions. Submission of form AD148 to the Career Management Agency supersedes all previously submitted posting management information or preferences.

Purpose of the AD148

This form will be used by all Full Time and Reserve members to record their career preferences. It will be used extensively by Career Managers and will contain information such as Preferred Locality and Undesired Locality, Unit Preference and Employment Preference, Posting Considerations, Personal Priorities and Reservists Availability.
