National ADF Family Health Program

The National ADF Family Health Program commenced on 1st January 2014 following on from the success of the ADF Family Health Trial.

To register for the National ADF Family Health Program, please submit a completed AD858-1 (PDF) to the address on the form.

If you are having dificulty opening the above file, please use this version of the AD858-1-Printable (PDF) that can be printed and completed offline.

Registrations will be accepted at any time - there is no deadline.

Did you know?

"ADF Family Health is happy to accept claims for reimbursement (manual claims) for up to 12 months after the date of service - provided you were participating in the trial at that time".

When submitting manual claims for payment, where there is an associated MBS Item Number and Medicare Rebate, There is a requirement to send a copy of the Medicare 'Statement of benefit and Payment' with your claim form. These are available from the service provider or from a Medicare Office. Unfortunately a Medicare Claims History print out is not sufficient as it does not include enough detail to substantiate the claim.

Please Note

During December 2013, our contracted administrator Navy Health Ltd will commence sending Welcome Packs and ADF Family Health Cards for those who have successfully registered for the National ADF Family Health Program.

Please be advised that these will be sent in Navy Health branded envelopes.

If you do not receive your card before 1st January 2014 please keep any healthcare receipts for manual reimbursement.
