Defence Work Health and Safety Incident Reporting for non-DRN users

Form AC563:Defence WHS Incident Report, is the notification and reporting form for all WHS incidents in Defence. This form must be completed even if a DISCON signal (FATALCAS and NOTICAS) has been raised.

The form is to be used by all ADO employees (ADF, Civilian and Reservists) as well as ADF Cadets, contractors and visitors. Full details about reporting are contained on the front page of the AC563.

For instructions on how to submit an incident report when you do not have access to the Defence Restricted Network, please see below:

  1. Extract the files contained in the following compressed folder to be able to continue with Steps 2 - 5.

  2. For you to be able to open the AC563 form template (as per Step 3) you need to install setup.exe (Windows) or DTeF501_Install.dmg (Macintosh) found within the zip file above.

  3. Once installed, open the AC563 form template (AC563.itp) also found in the zip file above.

  4. Complete the AC563 form (AC563.itp) with details of the Defence WHS incident.


  5. Save the completed AC563 form for your records and email the completed form to Defence WHS Incident Reporting (ac563@defence.gov.au).
